Describe The Appropriate Management For The Blood Pressure Fluctuations Seen In Gbs
Blood pressure fluctuations in GBS are best monitored in a medical intensive care unit setting. Adequate administration of isotonic IV fluids should be started, a bladder catheter should be placed, fluid intake and output should be carefully monitored, and blood pressure should be measured frequently. Most importantly, continuous electrocardiographic monitoring is mandatory.
Pressor drugs may be necessary. These must be used with extreme caution because hypersensitivity responses are common. The use of short-acting agents such as dopamine or phenylephrine is recommended. There is often a delay of several minutes in blood pressure responses to pressor drugs. Treatment of paroxysmal hypertension in GBS should be avoided, unless end-organ damage is present. If antihypertensive agents are used, it is best to use short-acting agents that can be titrated to the blood pressure response. Again, hypersensitivity with resultant hypotension is a potential complication. There may be an exaggerated hypotensive response to even small doses of IV drugs .

Blood pressure varies throughout the day and is influenced by a number of factors, says Dr. Joshua Penn, a cardiologist with Cedars-Sinai Medical Group and in private practice in Beverly Hills.
For starters, all humans have a natural daily rise and fall in blood pressure that corresponds with their circadian rhythm.
For most people, blood pressure will be at its lowest in the early morning hours and then rise through the late morning and peak in midafternoon.
Typically, this range will be about 10 to 15 millimeters of mercury on the upper, or systolic, value, which represents the peak pressure in the arteries and five to 10 millimeters of mercury on the lower, or diastolic, value, which represents the lowest pressure at the resting phase of the cardiac cycle.
Thus, a person with a resting blood pressure of 125/70 at 3 a.m., might have a reading of 140/80 by late afternoon.
Other factors contribute to fluctuations in blood pressure, but the most common one is hypertension, a condition characterized by chronic high blood pressure.
One of the manifestations of poorly controlled blood pressure is a high degree of variability, he says. A person with a lot of variation could easily have a blood pressure of 140/90 in the morning and a reading of 200/100 later in the day.
Respiratory Blood Pressure Fluctuations
Previously and incorrectly referred to as vagus in the polygraph field, RBPF represents the influence of breathing on the cardiovascular system. RBPF is manifested on the charts as a cyclical rise and fall of the blood pressure tracing, typically accompanied by constrictions of the pulse amplitude, that match the pace of the examinee’s breathing, but delayed by about 3 s. RBPFs can be caused by physiological factors such as cardiovascular health, age, obesity, medications, or fatigue. They can also be induced if the examinee overrides his homeostatic mechanisms by taking over breathing voluntarily in a manner that does not match his body’s requirements, typically breathing too deeply or too slowly or both. Voluntary breathing should be discouraged if observed, as it significantly reduces the diagnostic value of the respiratory data channel, and it can also induce very noisy blood pressure and electrodermal patterns.
Except when RBPFs are caused by over breathing, there are few options available to the examiner to dampen them. If fatigue is suspected as the cause, simply allowing the examinee to sleep before the next polygraph session can remedy the problem. If the examinee’s prescriptions are a possible cause of the RBPFs, the examiner must not tell the examinee to discontinue his medications, but must simply do the best possible to conduct the examination.
