Some patients have told me they experience a hangover or brain fog effect when they take melatonin. A few who suffered with depression said they felt more depressed. Dr. Reiter suggested that in those cases, to use a dosage of less than 1 mg, such as 200 mcg.

Dr. Reiter noted that in clinical trials with melatonin, you never see any mention of such unusual responses, which may also include hyperactivity. He attributed this to a potential reaction with other ingredients in the pills that are available commercially. Researchers, by contrast, use only pure melatonin in their studies. In spontaneously hypertensive rats , exogenous melatonin administration ameliorated hypertension as evidenced by reductions in mean, systolic and diastolic BPs. Melatonin treatment in hypertensive models has also been associated with reduced interstitial renal inflammation as measured by infiltration of lymphocytes and macrophages, enhanced endothelium-dependent vasodilation, improved cerebral autoregulation, sympathetic inhibition, reduced catecholamine levels and improved baroreflex responses.

In models of type 2 diabetes with insulin resistance hypertension and hypertension in rats with metabolic syndrome, exogenous melatonin administration significantly ameliorated hypertension.

  • Doctors have little time to examine patients and truly get to know their needs.

In my research, Ive read a good deal about melatonins potent antioxidant properties. I asked Dr. Reiter for his take on how these properties could benefit the cardiovascular system.

He sees melatonin as working on multiple levels. First, he said, melatonin is quickly distributed throughout the body. It enters cardiac cells with ease and has the ability to cross the blood brain barrier. Most importantly, melatonin gets inside the mitochondria, the power plants within cells where energy is produced. Few nutraceuticals can penetrate to this point, which makes melatonin especially useful.

A huge number of free radicals are produced as a byproduct of the mitochondrial energy-production process. Free radicals, as a reminder, are molecules with unpaired electrons that snatch electrons away from other molecules, setting off a biochemical chain reaction that potentially leads to inflammation and damage to cells and tissue. Unless the body controls free radical activity, the damage can be extensive even deadly, over time. Melatonin helps scavenge for, and neutralize, free radicals. It also has an anti-inflammatory effect.

We think these protective actions have potential applicability for individuals with cardiovascular disease, Dr. Reiter said.

Dr. Reiter added that in animal experiments, melatonin has been shown to sharply reduce local tissue damage caused by strokes and heart attacks.

The report appears in the February 2004 issue of Hypertension.

Lead author Dr Frank AJL Scheer told heartwire that they hope to pursue further studies of melatonin to see whether its salutary effects on blood pressure might be useful to improve both blood pressure and sleep quality in patients with borderline hypertension or whether it could be given in addition to antihypertensive medications. “That’s an open field, I think, that deserves more study,” Scheer said.

The day-night amplitudes of the rhythms in systolic and diastolic blood pressures were increased by 15% and 25% respectively, but there was no effect on heart rate.

Because of its ability to promote sleep, melatonin should be taken before bedtime. recommends taking 0.3 to 1 milligram of melatonin an hour and a half before bedtime. The website warns that taking too much melatonin — more than 1 milligram — may have an adverse effect and end up disrupting your sleep cycle. What Are Melatonin Side Effects

Melatonin needs vary from person to person. On average, most people need less than 3 milligrams per night to sleep better. Yet some people take more.

Melatonin is generally safe, but some people are more sensitive to the oral hormone and experience side effects with use. Adverse effects tend to occur with long-term use or when a person takes too much.

The dietary supplementation of melatonin appears as an effective option to compensate the physiological decline in the production of pineal gland melatonin with ageing however, the role of dietary melatonin in health and disease is still not well established . The strong limitation for the studies of dietary melatonin is the lack of standardized methods to determine melatonin concentration in foods with adequate sample treatment to obtain accurate results, thus making some of its proposed benefits hard to swallow . In addition, most of the studies on dietary melatonin measure melatonin content in food, but very few evaluate the bioavailability of melatonin after melatonin-rich food consumption. It is well established that the bioavailability of melatonin after oral and intravenous melatonin administration in human is very low with a maximal half-life of 10â60 min . Although other factors may also influence the bioavailability of melatonin which is mostly metabolized by the enzymatic catabolism in the liver . As shown in the Figure 4, melatonin is principally metabolized in the liver by cytochrome P-450 isoforms in 6-hydroxylation to yield 6-hydroxymelatonin which is thereafter conjugated with sulfates to 6-sulfatoxymelatonin and eliminated in urine . Melatonin may also be transformed to a less extend by CYP2C19 and CYP1A2 mediated O-demethylation in N-acetylserotonin or by deacetylation to 5-methoxytryptamine which can be reconverted to melatonin .

Results of a recent study suggest that melatonin may have a beneficial effect on blood pressure. The study, published in the Jan. 20, 2004, rapid-access issue of Hypertension, showed that taking melatonin at bedtime for three weeks lowered blood pressure among 16 men with untreated hypertension. However, a single dose of melatonin had no effect at all on blood pressure.

The researchers from Harvard Medical School and Brigham and Womens Hospital in Boston theorized that melatonin may affect blood pressure via its effect on the bodys biological clock. But since taking melatonin also improved sleep among the studys subjects, the researchers havent been able to rule out the possibility that sleeping better might also have helped with blood pressure reduction.

While these results are interesting, theyll have to be confirmed in larger studies before we can be sure that melatonin really does lower high blood pressure. If so, melatonin may be used in addition to or instead of medications now used to control blood pressure. And, of course, studies will be needed to determine how melatonin interacts with the various drugs currently used to treat hypertension.

Cant Sleep Dont Turn On The Light

Darkness not sleep, as many people think is what causes the body to produce melatonin. Exposure to light, including nightlights, short-circuits this process.

Still, its difficult to wean yourself entirely of light at night. Thats why its best to use red bulbs in nightlights. Red light doesnt interfere with melatonin like other colors of light do especially the blue bulbs that sometimes come with nightlights.

Whatever you do, dont get up and read a book or open the refrigerator to pull out a snack. According to Dr. Reiter, if you do you that, you are producing the equivalent of jet lag. Even if you lay in bed in darkness and dont sleep, you still produce melatonin. If you wake up in the middle of the night and find you cant readily go back to sleep, take some melatonin a reduced amount but dont turn on the light.

Reduced Melatonin Levels Associated With Cardiovascular Disease

Clinically, melatonin is being increasingly recognized in the pathophysiology of cardiovascular disease . Low levels of serum melatonin as well as its urinary metabolite, 6-sulphatoxymelatonin have been reported in various CVD’s including: coronary heart disease, angina, congestive heart failure and myocardial infarcts.

3.1.1 Coronary heart disease

In a study of 15 CHD patients vs healthy controls, night time serum melatonin concentrations were found to be more than 5Ã lower in CHD patients than in controls. Similarly, in 16 patients with coronary artery disease , blood samples collected every 2 hours between 22:00 and 08:00 revealed that CAD patients secreted less melatonin at 2 am, 4 am, and 8 am compared to controls. In 48 male CHD patients vs 18 matched controls, overnight urinary aMT6s levels were also significantly lower in the patient group. Finally, in comparing urinary aMT6s levels in patients with stable vs unstable angina, coronary disease and healthy controls, patients with unstable angina had significantly lower nocturnal urinary aMT6s levels compared to both healthy controls and patients with stable angina. In contrast, these lower levels were not significantly different from patients with CD. The results suggest that low melatonin production is associated with higher risk of cardiac infarction and/or death, as is the case with unstable angina.
