Preventing High Blood Pressure

Blood Pressure During Pregnancy

To keep your blood pressure in the normal range, your daily habits are key. These things help:

Donât smoke. Among the many health problems that smoking causes, it raises your blood pressure.

Make physical activity a habit. Most experts recommend at least 30 minutes of moderate-intensity physical activity five or more times a week. Or you could do a harder activity for a shorter period of time per session.

Eat right. Read food labels to see how much sodium is in a serving. Check with your doctor to find out what your daily limit should be. Include a lot of vegetables and fruits, along with whatever else you choose to put on your plate.

Stick to a healthy weight. Extra pounds raise your blood pressure. If youâre not sure what a healthy weight would be for you, ask your doctor.

Get enough sleep. For most adults, thatâs 7-8 hours of sleep per night, on a regular basis.

If you drink alcohol, limit it to no more than one drink a day if youâre a woman and up to two drinks a day if youâre a man.

What Are Possible Complications Of High Blood Pressure In Pregnancy

High blood pressure taxes the blood vessels, which in turn decreases blood flow to the liver, kidneys, brain, heart, lungs, uterus and placenta. When gestational hypertension isnt under control and the placenta isnt getting enough blood, fewer nutrients and oxygen make it to your baby, which can slow fetal growth.

And if gestational hypertension morphs into preeclampsia, premature delivery or placental abruption can occur, which is the too-early separation of the placenta from the uterus. In the worst of cases, stillbirth and eclampsia are possible. Untreated very high blood pressure can also lead to stroke.

High Blood Pressure And Pregnancy

High blood pressure, or hypertension, does not usually make you feel unwell, but it can sometimes be serious in pregnancy.

Your midwife will check your blood pressure at all your antenatal appointments.

If you are pregnant and have a history of high blood pressure, you should be referred to a specialist in hypertension and pregnancy to discuss the risks and benefits of treatment.

If you develop high blood pressure for the first time in pregnancy, you will be assessed in a hospital by a healthcare professional, usually a midwife, who is trained in caring for raised blood pressure in pregnancy.

Home Blood Pressure Monitoring

Blood pressure tests can also be carried out at home using your own digital blood pressure monitor.

This can give a better reflection of your blood pressure, as being tested in somewhere like a GP surgery can make you feel anxious and can affect the result. It can also allow you to monitor your condition more easily in the long term.

You can buy a variety of low-cost monitors so you can test your blood pressure at home or while you’re out and about.

It’s important to make sure you use equipment that has been properly tested. The British Hypertension Society has information about validated blood pressure monitors that are available to buy.

Indication Of Blood Pressure Reading

  • More than 90/60, but less than 120/80: This states that you are having a healthy pressure and have an ideal condition
  • More than 120/80 and lower than 140/90: Have a normal reading, but still a bit higher compared to reading that it should be. So, take proper measures to lower it.
  • 140/90 or more: Having a very high blood pressure that indicates hypertension

Regular blood pressure examinations are very important during pregnancy. Most women may not develop high blood pressure during pregnancy. However, high blood pressure may develop in some women. Most often the increase is mild. But in some cases, high blood pressure may become severe and harmful to both the mother and her baby. Some women may even develop pre-eclampsia a more acute condition which is discussed below.

What Medications Are Used To Treat High Blood Pressure

Four classes of high blood pressure medications are considered first line when starting treatment. Sometimes other medications are coupled with these first-line drugs to better control your high blood pressure. First-line, pressure-lowering medications are:

  • Angiotensin-converting enzyme inhibitors block the production of the angiotensin II hormone, which the body naturally uses to control blood pressure. When angiotensin II is blocked, your blood vessels dont narrow. Examples: lisinopril , enalapril or captopril.
  • Angiotensin II receptor blockers block this same hormone from binding with receptors in the blood vessels. ARBs work the same way as ACE inhibitors to keep blood vessels from narrowing. Examples: metoprolol , valsartan or losartan.
  • Calcium channel blockers prevent calcium from entering the muscle cells of your heart and blood vessels, allowing these vessels to relax. Examples: amlodipine , nifedipine , diltiazem .
  • Diuretics flush excess sodium from your body, reducing the amount of fluid in your blood. Diuretics are often used with other high blood pressure medicines, sometimes in one combined pill. Examples: indapamide, hydrochlorothiazide or chlorothiazide.

Low Blood Pressure Symptoms

High Blood Pressure and Pregnancy

A Typical Night in the ER

Medical Author: Benjamin C. Wedro, MD, FAAEM Medical Editor: Melissa Conrad Stöppler, MD

The ambulance call comes in.

“We’re running lights and siren with a 67 year old male. He’s confused, lethargic, blood pressure 80 over 40, thready pulse and sweaty. We’ll be at your door in a couple of minutes.”

The nurses start setting up the equipment to care for the patient. We’ve been through this drill before when someone arrives at the ER with low blood pressure. I hope it’s an easy diagnosis and something that we can treat quickly. IV’s are set up, monitor pads ready. The respiratory tech arrives with her machines just in case they are needed.

The EMTS unload a man in obvious distress, sweaty, struggling to breathe, and limp.

“He’s had a fever for a couple of days with a cough. Past history of COPD, heart problems, still smokes.”

The diagnosis still could be easy, but there are a lot of things that could be causing this man’s condition. He is in shock his low blood pressure is not enough to provide blood flow and oxygen to his brain. That’s why he is lethargic. No doubt the rest of his organs are at risk too. But why?

If High Blood Pressure Becomes Severe Or If Pre

There are risks to both you, as the mother, and to your baby if high blood pressure becomes more severe, especially if you develop pre-eclampsia. You will usually be seen urgently by a specialist and you may be admitted to hospital. Blood tests may be suggested to check to see how much your blood pressure or pre-eclampsia is affecting you. The well-being of your baby may also be checked using ultrasound scanning. A recording of your baby’s heart rate may be carried out.

For severe high blood pressure, especially if pre-eclampsia develops, there is often a dilemma. If the high blood pressure is caused by the pregnancy, the only cure is to deliver your baby. This may be fine if your pregnancy is near to the end. The birth can be induced, or your baby can be born by caesarean section if necessary. However, a difficult decision may have to be made if high blood pressure or pre-eclampsia becomes severe earlier in your pregnancy.

Medicine to lower the blood pressure may be prescribed for a while. The most commonly used medicine is labetalol. This may allow your pregnancy to progress further before delivering your baby. The best time to induce the birth varies depending on the factors mentioned above.

If you have severe pre-eclampsia, the medicine magnesium sulfate may be given via a drip around the time that your baby is delivered. This may reduce your chance of developing eclampsia and prevent you having fits .
