The goal is to reduce your risk of developing lymphedema. Having a mastectomy alone does not put you at risk for lymphedema. The risk comes when you have lymph nodes removed or damaged. If you had any of the following, you should take precautions to reduce your risk of lymphedema:
- Removal of all lymph nodes in the underarm area, which is called an axillary node dissection .
- Removal of a limited number of key lymph nodes from the underarm area, which is called sentinel lymph node biopsy .
- Having radiation to the chest wall and armpit after surgery.

How To Take Your Own Blood Pressure Using A Battery
Digital blood pressure machines are nearly as accurate as air-driven ones. If you are using a digital blood pressure device, follow these instructions. Note that a digital blood pressure device should be checked for accuracy every 6 months, comparing results with your doctor’s mercury-driven machine.
|1. Apply the blood pressure cuff snugly onto your upper arm about one inch above the crease in the bend of your arm. Velcro closure may be released and reset to tighten cuff as needed.|
|2. Sit quietly with your arm straight, palm up and your elbow resting in a relaxed position on the table. Your upper arm should be about the same level as your heart.|
|3. Turn on the machine by pressing the “ON” button. Wait for the blinking heart to appear in the read-out screen, then press the “START” button.|
|4. The cuff will tighten, then slowly release. In less than 60 seconds, your digital blood pressure reading will appear in the read-out screen.|

Choosing A Home Blood Pressure Monitor
The American Heart Association recommends an automatic, cuff-style, bicep monitor.

Check The Cuff For Correct Sizing
The Welch Allyn cuff has an arrow on it labeled index. Many people think the arrow is supposed to point down your arm towards your hand, but its not. The index arrow is supposed to point up towards your shoulder.
The purpose of it is to check the size of the cuff and make sure its the correct size for your arm. The Welch Allyn cuff has a size range indicator line. If the cuff is the correct size, when its tightened and secured, the arrow will point somewhere on the line.

What Your Blood Pressure Reading Means
Blood pressure readings include two numbers. The top number is called the systolic blood pressure and it changes with every heartbeat, says Dr. Del Conde. When the heart contracts and pumps extra blood into the circulatory system, the pressure increases. This increased pressure corresponds to the top number, or the systolic blood pressure.
The bottom number is called the diastolic blood pressure. When the heart is at rest and not contracting, the pressure that exists within the system is called the diastolic pressure, says Dr. Del Conde.
Lining Up The Artery Marker
The artery marker symbol on the Welch Allyn blood pressure cuff is labeled artery. The symbol is a small circle with a straight line drawn through the middle of the circle. The line through the circle should line up with the brachial artery on the upper arm.
The brachial artery runs down the inner part of your upper arm facing towards the side of the torso. It then crosses over the bend off your arm towards the top of the forearm. The brachial artery is what the nurse most often uses when drawing blood from your arm.
The reason why the artery marker needs to line up with the brachial artery is how the artery gets compressed. When the BP cuff inflates, it squeezes the upper arm and stops the flow of blood through the artery.
To cut off the blood flow efficiently, the middle of the air bladder inside the cuff should be as close to the brachial artery as possible. The artery marker helps accomplish that task.

Youve Placed The Cuff Incorrectly
The standard for blood pressure cuff placement is the upper arm using a cuff on bare skin with a stethoscope placed at the elbow fold over the brachial artery.
The patient should be sitting, with the arm supported at mid heart level, legs uncrossed, and not talking. Measurements can be made at other locations such as the wrist, fingers, feet, and calves but will produce varied readings depending on distance from the heart.
The mean pressure, interestingly, varies little between the aorta and peripheral arteries, while the systolic pressure increases and the diastolic decreases in the more distal vessels.
Crossing the legs increases systolic blood pressure by 2 to 8 mm Hg. About 20 percent of the population has differences of more than 10 mmHg pressure between the right and left arms. In cases where significant differences are observed, treatment decisions should be based on the higher of the two pressures.

- Sit with your back straight, your legs uncrossed, and your feet flat on the floor.
- Place the cuff on your left arm or wrist .
- For upper arm monitors, wrap the cuff around the middle of your bare upper arm, about ½ above the elbow. It should be fastened snugly, but you should be able to slide two fingers underneath the cuff. Place the air hose on the inside of the arm near the crook of your elbow so that the artery position mark is aligned with your brachial artery.
- For wrist monitors, with the palm facing upwards, attach the monitor to the inside of your wrist. Fasten securely with the top edge of the cuff aligned with the crease of your wrist.

How To Apply The Cuff Correctly

Wrap the cuff snuggly around the arm, on bare skin or light innerwear,making sure the cuff is securely in place. Remove heavy clothes such as a sweater before measurement, instead of rolling up the sleeves.

Adjust the position of the air tube so that it runs down the inside of your arm and is positioned at the center of your arm.

Position the cuff about 1-2cm above the bend of your elbow.Make sure that the cuff does not cover the joint region.

The cuff may be wrapped around either the right or left wrist. However, because measurement values vary between the right and left wrists, make sure to always measure on the same side.

Sensors estimate the Mean Arterial Pressure and the patients pulse rate. Software in the machine uses these two values to calculate the systolic and diastolic BP.

A normal adult MAP ranges from 70 to 105 mmHg. As the organ most sensitive to pressure, the kidneys typically require an MAP above 60 to stay alive, and sustain irreversible damage beyond 20 minutes below that in most adults. Because individual requirements vary, most clinicians consider a MAP of 70 as a reasonable lower limit for their adult patients.
Finally, and especially in the critical care transport environment, providers will encounter patients with significant variations between NIBP and arterial line measured blood pressure values.
