Red wine doesn’t lower blood pressure

We graded the overall certainty of evidence using the GRADE approach via GRADEpro GDT software we formulated summary of findings tables.

We created three SoF tables to show the certainty of evidence and the summary of effects on outcomes of interest for high , medium , and low doses of alcohol.

Ratings of the certainty of evidence ranged from moderate to low in this review, which suggests that the effect estimates of alcohol might be slightly different than the true effects. For high doses of alcohol, we found moderatecertainty evidence showing a decrease in SBP and lowcertainty evidence suggesting a decrease in DBP within the first six hours and 7 to 12 hours after consumption. Moderatecertainty evidence shows that SBP and DBP rise between 13 and 24 hours after alcohol ingestion.

For medium doses of alcohol, moderatecertainty evidence shows a decrease in SBP and DBP six hours after alcohol consumption, and lowcertainty evidence suggests a decrease in SBP and DBP for 7 to 12 hours after alcohol consumption. After 13 hours of consumption, SBP and DBP were raised the certainty of evidence was low and medium, respectively.

For low doses of alcohol, we found lowcertainty evidence suggesting that SBP, DBP, and MAP fall within the first six hours after alcohol consumption.

Myth: Red Wine Is Good For Everyone

The hormone AII is a potent vasoconstrictor that stimulates aldosterone and vasopressin secretion from the adrenal gland, promoting sodium and water retention . As a result, peripheral resistance and blood volume are increased, leading to elevated arterial blood.

Several clinical trials in humans and studies conducted in animal models have reported stimulation of the sympathetic nervous system and increased noradrenaline after consumption of alcohol . When noradrenaline stimulates the adrenergic receptors located in the heart muscles, heart rate and blood pressure are increased.

Another possible mechanism is the increase in plasma cortisol levels following heavy alcohol consumption . Several studies have suggested a role for cortisol in alcoholinduced hypertension . Cortisol is a type of steroid hormone, and the presence of excess cortisol has been associated with elevated blood pressure in normotensive individuals .

Expert Reaction To Study On A Molecule In Red Wine And Blood Pressure

Research published in Circulation demonstrates that a molecule found in red wine causes a drop in blood pressure.

Prof Roger Corder, Emeritus Professor of Experimental Therapeutics, Queen Mary University of London, said:

I have been pushing back for many years against associating resveratrol with any effects of red wine. In fact, most red wines have undetectable levels of resveratrol. I published correspondence in Nature in 2003 highlighting the divide between doses of resveratrol being used experimentally compared to the amounts present in red wine. But the urban myth that resveratrol might explain the health benefits of regular red wine consumption has persisted. The dose here of 320 mg/kg in mice can be considered equal to treating a human with 15 to 20 g resveratrol. This amount is ridiculous and in red wine terms equivalent to more than 3000 litres of wine.

A positive relationship between alcohol consumption and blood pressure is well established, but the relative effect of specific alcoholic beverages is controversial, said lead researcher Renate R. Zilkens, Ph.D., research fellow in the School of Medicine and Pharmacology at University of Western Australia.

Red wine appears to have a dual effect on blood vessels. The alcohol in it may raise blood pressure, but the polyphenolic compounds in red wine may have antioxidant effects and help relax blood vessels. There may be subtle differences between wine, beer and spirits on heart rate and other cardiovascular measurements.

Researchers investigated whether red wine has less of an impact on blood pressure than other alcoholic beverages and whether it improved vascular function. They studied 24 healthy, non-smoking men aged 20 to 65. All the men were daily drinkers, with normal blood pressure and no history of heart disease.

Removing alcohol from red wine did not lower blood pressure either, Zilkens said.

Many Factors Drive Blood Pressure

But he pointed out that this is an observational study, so it cant show for certain the health benefits or harms of moderate drinking.

For that, youd need a randomized controlled trial, in which groups of people are assigned to drink a certain amount of alcohol each day for months or years.

For example, people who drink lightly or abstain may also tend to eat healthy, exercise regularly, and get enough sleep all of which can lower their blood pressure.

Grayver said the type of alcohol that people drink could also make a difference. Red wine, for instance, is thought to have anti-inflammatory effects on the body, which may also impact heart health.

And what you eat with your glass of beer or wine or shot of whiskey can affect your blood pressure, too.

All of the pretzels, peanuts, and other snacks that go along with alcohol consumption have a significant amount of salt in them, said Grayver. That in itself could be driving the hypertension.

Even news stories that talk about the heart benefits of alcohol may play a part.

It is possible that once someone receives a diagnosis of hypertension, they may then be incentivized to drink alcohol in moderate amounts, said Marcus.

In a 2015 study, Marcus and his colleagues found that people who believed that alcohol is heart healthy were more likely to drink more, compared to those who thought otherwise.
