Data Analysis And Statistics
BP and HR measurements were averaged across 24 hours and over the wake and sleep periods. Sleep and wake periods were defined as the actual time periods that the subjects were in bed attempting to sleep and when they were out of bed and awake, respectively . The cosine analyses were performed on actual clock time of measurement and included a condition factor and both fundamental and the second harmonic terms of the 24-hour day. Thus, the above cosine analysis tested the effect of condition and its interactions with the fundamental and second harmonic terms. Statistical tests were performed with SPSS for Windows, version 18 . Statistical significance was accepted at P< 0.05. Data are presented as mean ± SEM.

This lack of a consistent treatment effect may be related to multiple variables, including differences in study design, type and size of cohorts, degree of CPAP compliance, treatment duration, and accuracy of BP assessments.
Recently, four metaanalyses of randomized controlled trials on CPAP use have been published . Bazzano et al analyzed 16 randomized clinical trials published between 1980 and 2006, representing 818 participants, that compared participants treated with CPAP with control subjects, that had a minimum treatment duration of 2 weeks, and that reported BP changes during the intervention and control period. Mean net change in systolic BP for participants treated with CPAP vs control subjects was 2.46 mm Hg mean net change in diastolic BP, 1.83 mm Hg and mean net change in mean arterial pressure, 2.22 mm Hg . The authors concluded that their analysis provided evidence that effective CPAP treatment reduces BP.

Osa And Resistant Hypertension
OSA is common in patients with resistant hypertension, which is defined as BP that remains uncontrolled with three or more medications. In a prospective evaluation of 41 patients with resistant hypertension, Logan et al found that 96% of the men and 65% of the women had significant OSA . In 71 consecutive subjects referred to the hypertension clinic at the University of Alabama at Birmingham for resistant hypertension, we found that 90% of the men and 77% of the women had OSA . As OSA severity increases, there is an increased need for additional BP medications that is, the more severe the OSA, the less likely BP is controlled with pharmacologic therapy.- A prospective, but uncontrolled CPAP trial demonstrated that CPAP use can have substantial antihypertensive benefit in patients with resistant hypertension. Logan et al reported that CPAP use after 2-month follow-up in 11 patients with resistant hypertension lowered nighttime systolic BP by 14.4±4.4 mm Hg and diastolic BP by 7.8±3.0 mm Hg.
Lowering Systolic Blood Pressure More May Cut Health Risks
One major study found that lowering systolic blood pressure to well below the commonly recommended level also greatly lowered the number of cardiovascular events and deaths among people at least 50 years old with high blood pressure.
When study participants achieved a systolic blood pressure target of 120 mmHg compared to the higher target of 140 mmHg recommended for most people, and 150 for people over 60 issues such as heart attack, stroke and heart failure were reduced by almost one-third, and the risk of death by almost one-fourth.
“That’s important information, because more lives may be saved and more deaths may be prevented if we maintain lower blood pressure in certain patients,” says Lynne Braun, NP, PhD, a nurse practitioner at the Rush Heart Center for Women.
Braun cautions, however, that your personal blood pressure target depends on a variety of things, including your current blood pressure, lifestyle, risk factors, other medications you are taking and your age. “Every person has to be evaluated as an individual,” she says. “Realistically, we can’t get everybody down to 120, and trying to do so may create unintended problems.”
It can be dangerous, for instance, to keep an older person on medications that have unsafe side effects, such as diuretics , which can cause dehydration and dizziness in older adults.

“These results add to the mounting evidence that shows the importance of vascular risk factors in contributing to memory problems,” said study author Adam M. Brickman, Ph.D., of Columbia University in New York, N.Y. “They also point to the potential impact of preventing high blood pressure through efforts such as maintaining a healthy weight, being physically active and having a healthy diet.”
The study involved 435 people with an average age of 59 who were enrolled in a study of aging in Venezuela. Their blood pressure was monitored for 24 hours at home with a device that took their pressure every 15 minutes during the day and every 30 minutes at night. They had brain scans to look for the small areas in the brain that appear damaged from vascular disease, called white matter hyperintensities. They also took tests of their memory and other thinking skills.
